Harbor Care located in Nashua, New Hampshire seeks a compassionate and dynamic physician leader to serve as its next Medical Director for the area’s only healthcare for the homeless clinic, Harbor Care Health and Wellness Center. This is a unique opportunity to join an award-winning designated National Health Service Corps site recognized by the National Committee for Quality Assurance (NCQA) for Patient Centered Medical Home and Behavioral Health Integration.
Harbor Care is an innovative health and human services non-profit organization that provides housing, healthcare, home health, behavioral health, addiction treatment, and more in collaboration with many community partners throughout New Hampshire. With 325 employees, serving 5,000 patients/clients, Harbor Care offers many programs specifically focused on Military Veteran and homeless populations. The organization integrates stable housing with vital services such as: medical, dental, and mental health care, substance misuse treatment, employment services, and other wraparound support services proven to end homelessness, help change and save lives. Harbor Care has an annual budget of approximately 40 million dollars.
The Medical Director works as a member of Harbor Care’s administrative team and is relentless in the pursuit of a culture of providing extraordinary patient care and services to all community members regardless of ability to pay. They will demonstrate a strong advocacy for patient-centered care, excellent service, seamless care coordination, physician leadership and recruitment, as well as innovative and efficient care delivery.
It is critical that this physician leader establish themselves as a visible, approachable individual who is a transparent, proactive and effective communicator. They must be naturally orientated to, and personally invested in taking the time to understand the culture and build trusting, collaborative relationships. The Medical Director must be able to clearly and comfortably communicate and collaborate with physicians, APP’s and other patient care providers of various backgrounds and administrators at various levels, while formulating an approach to infrastructure and systems to underpin an integrated high performance group practice.
Establishing immediate credibility in ways that create trust and support among physicians and APPs will be critical for the Medical Director. Credibility will be established by visiting and carefully listening to providers and understanding their interests, goals and challenges, and by genuinely demonstrating authenticity, integrity, energy, and collaboration. This relationship will further enhance how Harbor Care can optimize performance, services, and standards as well as support providers to do their best and most meaningful work.
The ideal candidate will be a visible and engaging Board-certified physician, M.D., or D.O., with a strong clinical background and be eligible for medical and DEA licensure in New Hampshire. An advanced degree MBA, MMM, MPH is preferred but not required for candidacy.
