Roseville, California - Adventist Health has retained Witt/Kieffer to conduct a national search for their System Medical Director, Care Management (SMDCM). Ideally, this position will be based in Roseville, California, and will serve as the most senior care management physician advisor within this 19 hospital, four region $5 billion health system.
The SMDCM role represents the opportunity to join a mission-driven and complex organization that is continuing its transition from a federated model of care delivery to a more standardized and centralized model, focused on the entire continuum of care. This leader will walk into a nationally-recognized, multi-state health system that has substantial reach and can profoundly impact the health of the populations it serves. As the senior care management physician advisor for the health system, the SMDCM will be a keystone physician advisor in the care transformation process, in close collaboration with the system Chief Clinical Officer (CCO), Vice President Care Management (VPCM) and Regional Care Management Teams.
Reporting to the System CCO, and working closely with the VPCM, the SMDCM will be the subject matter expert (SME) in all Case Management related matters for all markets and regions served by the health system. She/he will collaborate with all regional care management teams to advance the health systems clinical agenda as it continues its journey from a fee-for-service based culture to one which is focused on fee-for-value. This leader will foster the clinical leadership and engagement necessary for care management transformation across the full continuum for the system.
The successful SMDCM will be a physician with appropriate board certification and progressive experience in clinical practice, including a background in care management, performance improvement and care redesign. He/she will have a minimum of five years practice experience and several years in a physician advisory leadership role; an advanced degree in health care or business related field is preferred. The successful SMDCM will be visible, proactive, decisive, an outstanding communicator, and able to build relationships across the region and organization to engage peers in delivering excellence in clinical care.
